Family Nurse Practitioner
Stephanie Salimbene works as part of the sub-acute rehabilitation team with Physician Specialists of Northern Jersey. Prior to completing her degree as a nurse practitioner, she worked as a Registered Nurse and was the Assistant Director of Nursing at Allendale Rehabilitation and Assisted Living Center in Allendale, NJ. Stephanie graduated from Chamberlain University with a Master’s degree as a Family Nurse Practitioner in 2021, after her prior studies there where she obtained her Bachelor’s of Science in Nursing as well.
Stephanie is currently practicing at CareOne at Ridgewood Ave Rehabilitation Center in Paramus NJ, where she provides care for patients requiring rehabilitation after their hospitalization. She also provides cardio-pulmonary and critical care consultations for patients who require closer monitoring of chronic illnesses, such as obstructive pulmonary disease or heart failure, in collaboration with Dr. Jason Shatkin and Dr. Michael Gross with whom she communicates specific needs or clinical changes patients may have. This transitional collaborative care helps prevent readmissions and improve individual patient outcomes.
Stephanie’s goal is to provide quality healthcare while also developing a bond with her patients and their families. She spends quality time listening to her patients and making every effort to understand the full aspect of their individual needs. She ensures clear communication with every patient and includes the patient’s family in the decision-making process so they can develop a plan that is mutually understood and agreed upon. She provides personalized patient care in addition to evidence-based practice guidelines.
