Telehealth Appointments
Our practice initially implemented Telehealth services when the COVID-19 pandemic occurred. We began offering virtual visits to our patients to avoid the spread of viral illness, especially for the safety of our elderly and immunocompromised patients, as well as ensuring our patients were still able to receive necessary care during the pandemic. We understand the many benefits of telehealth and continue offering this service to our patients today. Telehealth technology gives us the ability to consult with patients quickly and conveniently. Using HIPPA complaint applications with audio-visual conferencing technology, our physicians are able to speak to and visually visit with patients remotely, from the comfort of their own home or while in the workplace. This makes healthcare more feasible for those who have difficult work schedules or cannot leave their home for various reasons.
Virtual appointments can be done using a smartphone, tablet, iPad, desktop computer or etc. In cases where a physical examination with our on-site equipment is necessary for proper diagnosis and treatment (as telehealth has limitations), your provider will ask that you come into the office.
The goals and benefits of telehealth include:
- Make healthcare easier and quicker for patients to access
- Keep patients and medical staff safe from spreading viral illness or infectious disease.
- Make services more available for patients who are homebound due to immobility or other health reasons, and patients who live far away or who do not have available transportation.
- Providers are able to order diagnostic testing, discuss test results and prescribe medications during virtual visits.
Remote Patient Monitoring (RPM)
Our practice works with an outside chronic care management company to provide our Medicare and Managed Medicare patients over 65 years of age with Remote Patient Monitoring devices. The services include periodic phone calls from licensed Nursing staff to check-in on your health status, as well as distributing certain relevant medical devices, which are provided to patients in their homes to check blood pressure, glucose levels, pulse rate, oxygen levels and etc. The devices are provided to patients with chronic health issues such as diabetes mellitus, hypertension, COPD and many other conditions. These remote monitoring devices wirelessly transmit data back to our staff and alert us with abnormal readings. These readings aid in catching an early diagnosis of these chronic conditions as well as monitoring current status and progression of illnesses before they become critical. Chronic Care Management and Remote Patient Monitoring have been proven to significantly decrease patient hospitalizations and have also shown a decrease in patients being re-hospitalized after discharge.
